Thanks to a new grant opportunity, PathPoint is now seeking a full-time, bilingual, Transitions Outreach Coordinator for our PUENTE program! This position is limited, and funded through a 24-month grant from the Department of Developmental Services in partnership with Tri Counties Regional Center. This is a full-time position, 40 hours per week, some weekend and evening work required. 10-25% of the position requires travel throughout the counties of San Luis Obispo, Santa Barbara and Ventura. This position can be based in any of the three service areas with partial remote availability.

PathPoint partners with people with disabilities, people with mental health diagnoses, and young adults to pursue their hopes and dreams through strengthening workplace abilities, building life skills, and developing meaningful relationships. Founded in 1964, we offer services in five Central Coast, and Southern California counties: Kern, Los Angeles, San Luis Obispo, Santa Barbara, and Ventura.

Your impact as an Outreach Coordinator:

In this dynamic role, you will work 1:1 with individuals between the ages of 14-22, alongside their families to provide information and support as they plan to transition from school to adult services. A large portion of this job is spent in the community meeting with individuals to explore their options and plans for the future. The transition process can often be overwhelming and challenging for families; It is up to you to instill trust, and guidance, using a person-centered approach. The focus demographic includes individuals that use American Sign Language (ASL) to communicate, alongside Latinx, African American, Asian, and Mixteco identifying individuals. This role oversees our PUENTE Collaborative which includes supervising PUENTE team members and supporting the individuals and families sharing their transition experience. As an Outreach Coordinator you will also participate in community events such as job fairs where you will network and give informative presentations on developmental disabilities, the Regional Center system, and PathPoint services. This position also works closely with the Regional Center liaison to collaborate and share resources and training for referred clients.

What we're looking for:

  • Bilingual (English/Spanish) or (English/ASL) required.
  • A minimum of one-year experience working with individuals with disabilities.
  • Someone with strong interpersonal, public speaking, and presentation skills.
  • Highly motivated, self-starter, organized with ability to multi-task.
  • Culturally sensitive: aware of differences in employees based on cultural backgrounds; aware of one's own culture and how someone's background may lead to biases; has the skills and resources needed to work with employees from all different cultures and the ability to avoid making biased assumptions based on someone's culture.
  • Must have a valid California driver's license and satisfactory DMV record.
  • Must receive a criminal record clearance through the DOJ.

Compensation: This is an hourly, non-exempt position starting at $22.50 per hour. The pay range is $22.50-$23.50 an hour. The pay range is subject to change based on a variety of factors, and the maximum pay represents the wage potential based on attaining the highest level of position experience, talent, and responsibilities.
